Independent Living Program

Overview

A Pre Life Skills Assessment will be given at the beginning of the Program or within 60 days of a youths 16th birthday. The Assessment shall contain information from the child, caregiver, and case manager. An Independent Living Plan will be written within 30 days of the completed assessment by the Teaching Parents. The Plan will outline the strengths, limitations, resources of the youth and services provided. The Plan will be reviewed and updated quarterly.

Goal

Primary goal is to teach youth the skills with the necessary competency for them to live productively, self sufficiently and responsibly with other adults in their communities and workplaces.

 

Program

Eligible youth meet to complete the CTH Independent Living Program. The Summer Program lasts approximately 2 months and is weekly. The Fall Program meets for 8 months. They both enhance the programming the Teaching Parents are already using in their homes, including A Pre Life Skills Assessment*. Each month one of the homes is responsible for menu selection, food preparation and serving the class participants.

 

Fee

Included in the per diem.

 

Clients

Adolescent youth, 16-21 years old, in placement with CTH.

 

Curriculum

The CTH independent living curriculum is geared toward the 16 and over youth with an eye toward meeting ODJFS requirements. Daily living skills addressed include: Securing and maintaining a residence, Home & Money Management, Shopping, Utilization of Community services and systems, Accessing and utilizing transportation, Utilization of leisure time, Personal care, hygiene and safety, Time management, Enhancement of personal decision making & communication skills, Planning for job/career development, Securing and maintaining employment, Planning for ongoing and emergency personal health care needs, Building a positive self image, Development of positive support systems.*  Youth have the opportunity to learn while using role play, cooking, word games, videos, problem solving worksheets and homework. This class provides systematic skill development, yet is still individualized to meet each youth’s target goals.

 

Outcome

Using a CTH Post Independent Living Life Skill Assessment to measure gain of competency skills.
